CCM Application Checklist
COMPLETING THE CCM EXAM APPLICATION

STEPS TO COMPLETE THE CCM EXAM APPLICATION |

| STEP 1 | Download the CCM Certification Guide to learn about eligibility requirements, completing the application, the exam structure, exam preparation, scoring, certificates, fees and more! |
| STEP 2 | After confirming that you meet the eligibility requirements, if you have an existing account with The Commission, log in HERE. If you need to create an account, do so HERE. |
| STEP 3 | Once logged in, select "My Account" at the top of the page and then select "CCM Exam Application". |
| STEP 4 | Complete the CCM application in its entirety and then choose "Checkout and Make Payment". |
INFORMATION NEEDED TO COMPLETE THE CCM APPLICATION
If you are applying with a current, active, and unrestricted license or certification in a health or human services discipline, you will need to know:
- Name of license or certification
- Who issued the license or certification
- License or certification number
- State that issued the license or certification
- Date you became licensed or certified
- Expiration date of the license or certification
OR
If you are applying with a baccalaureate or graduate degree in a health or human services field that promotes the physical, psychosocial, and/or vocational well-being of the persons being served, you will need to know:
- Degree
- Area of concentration
- Name of institution
- Year of graduation
All applicants will need to provide:
- Current employment setting
- Employer(s), start and end dates to fulfill eligibility requirements
- Supervisor(s) name(s) (spelled correctly and listed the same way as listed in The Commission’s® database, and be mindful of maiden/married surnames)
- Supervisor(s) current email address(es) (If audited, they will be contacted via email to verify employment.)
