Describe the effects care transition will have on a person with Alzheimer's disease and related dementia (ADRD) and be able to identify when and why a transition in care may or may not be necessary.
Explain the prevalence of ADRD in different health care settings and the impact of this population on the health care system.
Utilize evidence-based guidelines to assist with the transition of care for a person diagnosed with ADRD to improve interdisciplinary communication, improve the quality of care, and minimize unnecessary transitions of care.
