Discharge Planning Across the Miles-Planning a Long Distance Transition of Care

The learner will be able to identify and communicate strategies that the case manager can use to prepare their patients for a long-distance transition of care.

The learner will describe 2-3 discharge planning strategies that the nurse, therapist, case manager or discharge planner can employ to plan for a safe and efficient discharge plan while managing the inpatient length of stay issues and reducing avoidable hospital days.

The learner will be able to recall and access a variety of resources and information that they can use when coordinating a repatriation.
